The images following show the settings you should use to fill in the form

The top section are drop-down boxes where you pick from several options or fill in information about the problem you are having.

The first drop-down menu "Product" you would probably select Feedback studio
The second menu is "User Type" where you will probably select Instructor
The third menu is "Access Route" where you should select LTI
The following field is asking for information about your problem; is it an Issue, Question or a Request
The next field is asking for a Full Description of your problem, give as much details as you can.

The bottom section of the form is information about you and which Institution you work for and an authentication "reCAPTCHA" process and Submition button

 After you have submitted the form you should get an acknowledgement of your request by email within less than 30 minutes normally.

A more detailed reply will probably take several hours depending on the complexity of your problem; possibly asking for further details.
